Distance Assessment

Below is an outline of what a distance assessment would look like:

1. Request an online assessment with the form below.
2. Fill out your contact information so I can get back to you ASAP.
3. A payment of ($100 + 15% hst = $115 total) is required either via e-transfer or credit/debit payment with card info taken over the phone.
4. You will recieve an email with a detailed symptomology questionnaire, which takes 45-60 minutes to complete.
5. Once complete, you will be prompted to send it back to me.
6. I will generate your customized analysis with an easy-to-read color chart and written description, along with your customized recommendations and pricing for any required supplements.
7. You can then send your supplement payment, with your mailing address, and I will place your order and have it delivered right to your door via UPS.
8. A follow-up will be scheduled in 4-6 weeks to tweak your regime, if need be, with my personal support offered anytime.
